Documenting a Disability

The Office of Disability Services determines reasonable accommodations to individuals based on the impact and functional limitations caused by the disability.

IEP or 504 plans will not be considered sufficient documentation unless accompanied by a current and complete evaluation.

Guidelines for Documenting a Disability

Chronic Health Illnesses and/or Mobility Impairments

Psychological/Psychiatric Disability

Sensory Impairments (Visual and/or Hearing)
