Alternate Testing Accommodation Request Form (pdf)
Alternate testing accommodations are provided through the Alternate Testing Center, 216 D.D. Dougherty Hall.
The Alternate Testing Program is a complimentary service provided for instructors who are unable to provide alternate testing accommodations outlined on students’ Accommodation Plans. Alternate testing accommodations may include a distraction-reduced testing environment, extended time, assistive technology, and/or alternate format, as well as other individually tailored provisions. Reasonable testing accommodations are based on current, comprehensive documentation of a disability which is reviewed and evaluated by the Director and/or Assistant Director for the Office of Disability Services (ODS).
The Alternate Testing Program provided by ODS ensures the security of the testing environment. All exams are kept in a secured filing cabinet in a locked room. Students are monitored during their exams and must sign an integrity statement. Upon completion, exams are returned to the departmental office prior to 5:00 P.M. of the same day or by the following morning. No materials, other than the exam and those indicated by the instructor, are allowed in the testing environment. Book bags, jackets, hats, cell phones, and other personal belongings are placed in a separate room during exam administration. All testing materials, including any scratch paper used, are returned to the instructor.
Students who arrive late to the Alternate Testing Center for their exam will have that time subtracted from the total allotted amount of time, up to fifteen minutes. After fifteen minutes, the student must have the instructor’s permission to enter the Alternate Testing Center via a Late Entrance Form.
Students who are eligible to receive alternate testing accommodations in accordance with an Accommodation Plan and who wish to utilize their testing accommodations must discuss their Accommodation Plans with their instructors. Together a student and the instructor must determine whether the instructor can provide the testing accommodations or whether the Alternate Testing Program will be utilized for the exam.
Students and instructors must complete and return the Alternate Testing Accommodation Request Form three (3) full school/working days prior to each scheduled exam. The Alternate Testing Accommodation Request Form is available from ODS or electronically at www.ods.appstate.edu.
Test can be sent via email to odsalternate.appstate.edu.
